I'm sure I can create something in Excel but don't want to reinvent the wheel if one of you has designed something good already. I'm looking for a document I can print and take to each game that will track my players' stats each game for things like:
goals
assists
penalty minutes
shots on goal
plus/minus
faceoffs taken/won
etc....
Does anyone have something they could share with me? Thanks.

This is my gameday tri fold.
I put the line groups and d pairings number's across the top(long side) of the Plus Minus grid and number 1 thru 13 down the side. By doing the line groups, it is easier to record +/-.
I do shots with slashes. Box score also.
I list my centers under the SOG box and do check marks, x, and circles(EJ)across the line.
#, penalty, time and minutes in the penalties section. Time is crucial (particularly with stacks and stop releases) and I should do an extra column for it.
I record G and A in the line up boxes for the player.
I tri fold this around a piece of plastic I cut to fit. Cardboard would work. My front is Lineup, my back is Box Score. My inside back is +/-.

It is a landscape form, and I can't really tell if this fixes the problems with the formats.
Here is the key to this- a lot of this info will come from the gamesheet. Obviously you don't have access to this during the game, but you can spend the entire game with your head in your tri-fold. It helps to delegate and duplicate. I have my backup keeper and D coach do shots against, independently. My forwards coach and backup keeper do shots for. My forwards guy and I do +/-. All three of us do penalties because sometimes I'm a little too distracted "discussing things" with the official to do my journal entry.
Your coaching style will dictate how useful this form is. I focus on +/-, draws, and events that measure my keys to the game (dump ins, opponents offsides, 7 sec breakouts, shot blocks at the blueline, etc). Truth is, as your kids learn that you coach to the stats, they will focus that much more on doing what you teach because they know it is measured. Stats are a pain, but using them properly is the fastest way to the ah ha moments for you and your players.
