Dman (QB) picks up puck behind net, 3 others swing back low (dots) or high (inside blue line) and pick lanes (2 outside / 1 inside) while 5th man hangs off the wall at far blue line.

Dman steps out to join swing. Can skate it up or use 4 different passing options. 5th man needs to come across blue line and half pick defenders.
